I'm the original owner, from my subscription to the Sub Pop singles club, form is attached. Green marble vinyl. Played only a couple of times.
Vinyl has been cleaned. Stored in plastic sleeve. Some ring wear on back artwork but otherwise in great condition. From pet free & smoke free home.
