I do my best to grade conservatively according to Goldmine standards (see a more detailed explanation at the very bottom of this listing.)
Sleeve EX+/NM - still covered in original shrink. Some *very* light wear at open edge, otherwise superb. No hint of ringwear or any other damage.
LP EX+/NM - Vinyl is glossy and free of any scratches or marks noticeable to the naked eye ... even at close inspection. I've not done any cleaning, so does have a (very) little bit of dust/lint on the surface, otherwise terrific overall.

Mint - A record that is absolutely perfect in every way (very rarely used.)
Near Mint (NM) - A nearly perfect record. NM records are shiny, with no visible defects. Writing, stickers or other markings cannot appear on the label, nor can any spindle marks around the hole. Major factory defects also must be absent. If played, it will do so with no surface noise. NM covers are free of creases, ring wear and seam splits of any kind.
Excellent (EX) - A good description of a EX record is “except for a couple minor things, this would be Near Mint.” EX records may show some slight signs of wear, including light scuffs or very light scratches that do not affect the listening experience. Slight warps that do not affect the sound are okay. Minor signs of handling are okay, too, such as telltale marks around the center hole, but repeated playing has not misshapen the hole. There may be some very light ring wear or discoloration, but it should be barely noticeable. EX covers should have only minor wear. An EX cover might have some very minor seam wear or a split (less than one inch long) at the bottom. Also, a EX cover may have some minor defacing, such as a cut-out marking. 
Very Good (VG) - A VG record is one that has quite clearly seen generous use, but still remains a very reasonable copy for continued play and enjoyment. A VG record will have more obvious and/or more flaws than an EX record. They will typically lack most of the original gloss found on a new copy, and groove wear will be evident on sight, as well as light scratches deep enough to feel with a fingernail. When played, a VG record will have surface noise and some scratches may be audible, especially in soft passages and during a song’s intro and ending ... but the noise will not otherwise overpower the music. They also may have minor writing, tape, or a sticker on the label. VG covers will likewise show obvious wear and evidence of handling. Ring wear in the middle or along the edges of the cover is typical, though it shouldn't be overwhelming. Deeper, or more creases may be visible. Seam splitting may be more notable, and could appear on all three sides ... though it shouldn’t be obvious upon looking. VG covers may also have some writing, tape, stickers, or other marks on the surface.
Good (G) - A G record will have more and/or more severe flaws than a VG record, but should play without skipping (albeit with plenty of surface noise.) A G sleeve should be complete and in one piece, but will be flawed to the point of distraction. 
Additional Note - I sometimes add a "+" to grades when appropriate (i.e. "VG+" for a record that falls somewhere between VG and EX.) And also sometimes use the grade EX/NM when it seems appropriate (I like to grade conservatively, and find it hard to grade NM unless an item is absolutely superb.)
