Matrix no. MCA-263 etched into vinyl and MCA-363 printed on jacket.
I am not a professional grader--I chose near mint for the vinyl because it is blemish free and plays beautifully. There is a little bumpiness which I believe could be from the glue on the outside of the paper on side 2. but it has zero effect on the vinyl. 
The original MCA printed sleeve has a 2" split on the back edge but otherwise very good.
The jacket (cover) is showing some normal wear, primarily on the outer edges but has no rips, stains or tears.
Overall, this is a very rare find and you should be happy with it.
