Cover: VG+. Shows minimal shelf wear. No seam splits. No writing. Spine is nice. Solid cover.
Vinyl: I played the whole album listening through headphones. Plays cleanly, like a brand new album. Visual grade is NM. Looks like it was never played.
Includes the insert.
I grade conservatively and will mention any flaws to the cover or vinyl.
