Albert Ayler - "New Grass" - LP. 1974 Impulse Reissue (blue/green labels). 
VG+ Vinyl sounds excellent. VG Gatefold Sleeve with minor wear around top edge, some minor tears on front and back cover, notch cut (see pics). Seams are intact.
