I received this record as a child in the late 50's to mid-60's and have always stored in its cover in a smoke-free home. I played it quite a few times for the song "Zip-A-Dee-Doo-Dah".

It doesn't have an additional paper record sleeve nor do I remember one.
It does have its original metal 45 rpm adapter that allows it to fit on a stereo meant to play larger records.
The cover is in overall decent condition, although it does show some expected vintage wear, including some cracking of the top and bottom righthand edges of the jacket. See photographs and/or contact me for further details.
The record itself is in decent condition although I can see some slight scratches using a magnifying visor.
The best part: I just played both sides, and my husband and I both thought it sounded pretty good.
There is some slight crackling throughout in the background, but this wasn't a great detraction: we could clearly hear the words, singing, and different instruments in the orchestra, including the kazoos in "Zip-A-Dee-Doo-Dah."
