MY RECORD GRADING GUIDE: Mint (M): New/Sealed. Absolutely perfect in every way!  
Near Mint (NM): A nearly perfect record that has been opened and possibly played once or twice - no obvious sign of wear. The outer sleeve has no creases, folds, seam splits or any other noticeable defect.The same is true of any other inserts, such as posters, lyric sleeves etc. Basically looks close as new as the day it was bought.
Excellent (EX): Just a hint below a NM grading. Record has minor signs of use, but still in remarkable condition. Most wouldn't see the difference between a NM and and EX grade, as both look and sound fantastic. EX just means that the record may have been handled and played more than a couple times, but with great care by the previous owner. The same goes for the outer sleeves. EX graded sleeves will look great, will be very solid with NO splitting nor defects of any kind. Some extremely minor corner bends or outer edge wear may be visible, but only under close inspection.  
Very Good Plus (VG+): Shows some signs that it was played and otherwise handled by a previous owner who took good care of it. Record surfaces may show some slight signs of wear and may have slight scuffs or very light scratches that don't affect one's listening experience. Picture sleeves and LP inner sleeves will have some slight wear, lightly turn-up corners, or a slight seam-split. 
Very Good (VG): Many of the defects found in a VG+ record are more pronounced on a VG disc. Surface noise is evident upon playing, especially in soft passages and during the song's intro and fade, but will not overpower the music otherwise. Groove wear will start to be noticeable, as will visible surface scratches that may affect the sound but should not cause the record to skip. Sleeves will be more worn that a VG+ grade, with moderate to heavier wear/age, splitting and creases. 
Very Good Minus (VG-) The minor to moderate defects of the VG graded record (above) is now more noticeable. Noise between tracks may be more apparent and records may have light scratches deep enough to feel with a fingernail that may affect the sound. Labels may be marred by writing, or have tape or stickers (or their residue) attached. The same will be true of picture sleeves or LP covers.
