This record has a rare white label (see photo), all other labels are black. I've contacted Clearview Records and The Queers USA Band website and they've never seen one.

I got this at a Queers show in 1995 in Boston, they were handing them out for free. Comes in a plain white paper sleeve with cardboard. Another version was included in Punk Planet magazine #9 Sep/Oct 1995 which was square with the round record in the middle.
