Conditions:

Jacket/Sleeve: VG+ (light shelf and seam wear, no seam splits) 

Vinyl: VG (nice gloss, a few light surface scuffs which cannot be felt, there are lines and spots from long term storage in an old paper sleeve which do not effect play, there is also a very slight warp that does not effect play, plays GREAT on my turntable with occasional very light surface noise but overall plays very nice and quiet)
