Brand new factory sealed! Small piece of shrink wrap missing at 3 of 4 corners, the 4th corner a larger piece of shrink wrap missing from bottom right corner, causing some light wear on all corners, edges all in very good shape and sharp. See photos of missing shrink wrap. Other than that cover is conservative VG (imho), and record is mint never played. Cannot verify which of the ‘C’, ‘O’, ‘D’, or ‘E’ the inserts are.
