Record printed on thick card stock. Approx 5" by 7". Meant to be used as a postcard! 
Good condition. I have tried to play this on an inexpensive home record player, and the record plays fine with a few occasional sound bumps. Not sold as new or in perfect condition; perfect for a completist!
