This appears to be a test press of the US Warner Bros promo 12" (PRO-A-3753) although it does not include all of the runout etchings present on that version.

Visual grade vinyl: STRONG VG due to some light hairlines, very close to VG+, fully play-graded with no issues
Sleeve: Will ship in what is presumably the original white generic sleeve which includes hand written artist / title / date notes
