Here, as noted by the hole punch in the upper right corner, is a promotional copy of the LP "Yellow Submarine". This type of promo was given by sales reps to radio stations and/or record shops. Side One features songs from The Beatles while Side Two has music from the movie's soundtrack composed and conducted by George Martin. Of course, the last track on George's side is his arrangement of the movie's title song which is credited to Lennon-McCartney.
Originally released in 1969 this issue comes from 1971 as noted by the "mfd by apple records" rim print located on the A-side. The etched matrix numbers are Side One - A29 #2 Side Two - A29 #3
Now we get to the part about the condition of the cover and record.
As shown in the photos the cover shows wear along the side and bottom. The record has been played more than a couple of times. After cleaning it I played it and made a few notes. Side One has a small tick between tracks 2 and 3; there's a pop at the tail end of "Hey Bulldog"; 4 consecutive pops at the end of "It's All Too Much". Side Two has what I consider to be a lengthy scratch shown in photo 13 and it makes its presence known starting at the end of track 2 ("Sea of Time & Sea of Holes") and goes all through track 3, "Sea of Monsters". The truth is, this is a scratch and not just dust or dirt. So there you are.
