It may not be in one piece, or very close to falling apart- or may show signs of being mended back together with tape, glue, etc. Staining and ringer may obscure text and images, tearing and heavy wear will be evident.  A record will have issues that affect playback, may be moderately to heavily warped, and will show heavier or a very high number of scratches, scuffs, and other defects. The record could have areas of damage that render it unplayable in those areas.  F: A sleeve might be in multiple pieces, or heavily repaired, and will show heavy damage and wear.  F: A record will likely have areas where it is unplayable due to damages, defects, etc.
