According to "U2 Collector's Guide" (K2 Publishing, 1985) this is taken from the Warner Bros. Music Show radio only promo LP known as

Originally recorded live at the Paradise Theater, Boston, MA, March 6, 1981. The cardboard sleeve is a white generic sleeve. The "cover" is paper and wraps around the sleeve, but is not attached to it. I believe it never was. (Will come with a fresh plastic protective cover.) The paper sleeve shows some ear on the top left (pictured)

The record itself is pristine. Probably only played once, if ever. The label on the vinyl is blank.
