Both the vinyl and jacket are in very good plus condition. Original insert. Bumped corners/slight wear(creases): See Photos. Vinyl put in a new generic sleeve and, for shipping purposes, placed outside the jacket. Jacket and vinyl placed in a poly outer sleeve. Vinyl visually inspected for condition. .  **The presence of a hole punch and/or sawtooth cut indicates that the record (especially during the cd heyday) was a return to the label or that the label allowed the record store to hole punch/sawtooth cut the cover to allow the store to reduce/remainder the record.

Mint= Sealed, original shrink packaging. Note: Since the vinyl record is still sealed, it is  presumed to be in Mint condition.
Near Mint (M-): In excellent condition . Vinyl might have only a few minor surface scuffs and/or a minor scratch (no feelers).  In many cases, still in shrink other than the opening. Usually the vinyl played once or twice. 
VG+: Very good plus. Vinyl record has a few surface scuffs or hairlines. Any scratches are to be minor and not be feelers. 
VG: Very good. Vinyl record has many/numerous scuffs and/or minor hairlines scratches but none are feelable.
Good or  Good Plus: The vinyl record still plays without a skip but has significant surface noice and/or groove wear. (I call crackling or pops) . 
Records that are graded as Good or Good Plus can be enjoyed (without skipping) as holder copies (adding one under the assumption that at some point in time the collector will find and upgrade another copy).
