A vinyl LP set produced on the Prestige Record Label, Cat No - P-34004.
Internet search reveals this to be a first US pressing having P34004ARE2-1B (eyes with eyebrows) FML722771 DT A C40/30 CRPSM#2 SX. on the runout of side one, P34004BRE2 -1A (eyes with eyebrows) FML72277 DT A CRPSM#2 SX. on side 2, side 3 P34004CRE2-1B (eyes with eyebrows) FML72277 TD A C40/30 CRPSM#2 SX. side 4 P34004DRE2-1A (eyes with eyebrows) FML72277 DT A C40/30 CRPSMT#1SX. side 5 P34004E PE2-1A (eyes with eyebrows) FML62277 DP A C40/30 CRPSMP#1 SX. and side 6 P34004F - (eyes with eyebrows) B FML62477 DT A CRPPIT#1 SX.

The vinyl is unplayed, has minimal paper storage surface blemishes - I have graded the vinyl as Near Mint, see grading system below.   The trifold sleeve is original, has minimal storage edge wear - I have graded the sleeve as Excellent+. 
The inner sleeves - graded as Excellent.

Record Grading - Please read :
Vinyl Grade - Please read.
MINT - Possibly unplayed - excellent condition through out!
NEAR MINT (NM): This record has the lightest of light surface scratches on the vinyl. It should play without any problem.   EXCELLENT (EX): Minor scuffs which are visible. There may be more than a few scuffs. It should play without any problem!
VERY GOOD (VG): Record shows wear and / or surface scuffs. The vinyl still has great luster, but the flaws will be noticeable to the naked eye. The flaws may cause minor surface noise.
GOOD (G) Record has visible scratches / scuffs but plays without jumping / skipping.
FAIR (F) Record may jump or skip but worth having in the collection.
Cover & Sleeve Grade - Please Read.
MINT (M): No stickers, address labels, writing on the covers or labels. No tears or seam splits. No wear to the cover. 
NEAR MINT (NM): The cover looks as close to perfect with only minor signs of wear and/or age.
EXCELLENT (EX): Artwork is still fair. Some impression to the cover (minor outer ring wear),  but no ink wear! Some creases to the corners, but not wrinkled and obtrusive to the eye.
VERY GOOD (VG): The artwork looks OK with slightly more aging. The back of the cover usually gives away the age of the cover. Flat white paper will be somewhat yellow, yet no stains or mildew from water damage. Some wear to the seams or spine, but no tears or holes popping through.
GOOD (G) Covers may have edge damage, may have creases in more than one place and yellowing.
FAIR (F)  Covers have tears, edge and opening damage, ink marks / writing.
