Side 2 Blue Note records (sticker) . 161 Lexington Ave NYC   . In wax RVG BN - LP 1577 sides 1 and 2 with ear . 

No R or INC on each sides. 

 The  Record is Near Mint , just sticker record shop on side 2 and the cover is also  Near Mint , no seam slipts , note owner green signature in middle cover on back cover  . Jacket address 43 West 61st St , New York 23.  Please see scans for tracks listing.
