I do not grade my records or jackets as i am not able to play each one all the way through. However, i try to describe them as best i can and take as many photos of the front and back covers, and the labels of each record to help with identifying which pressing it is. I also have begun to add short videos of me placing the needle at the beginning of the record for a few seconds to get a ballpark idea of condition/noise level.
