Condition of cover:   NM-  Stock copy in shrinkwrap!   No delete holes or cut corners.  Shrink has been taped at edges decades ago, however, the tape peels easily away from the cardboard, leaving barely a trace on the cover.    Removing shrink will reveal a NM cover with only the slightest of flaws.  Three corners sharp, one slightly dented.   Small stripe of tape residue on back cover where torn shrink was repaired. 

Condition of disc:  EXC+/NM-  Clean disc, nearly as new.  Faint sleeve marks only and a couple of dull marks not affecting play.   Have play tested, and sounds great.
