Up for auction here we have a copy of The Velvet Underground & Nico, Andy Warhol - Catalog # V6-5008. Based on my research it appears to be a 1968 pressing based on catalog number and blue labels. Matrix info is:

The gatefold jacket is in good condition overall, I have not cleaned it. It does have some minor issues please look at the photos closely. There is no inner sleeve.
The stereo album has clean, clear bright blue labels, and the vinyl plays through but with significant crackle and pops throughout. I would personally not use this as a copy I would listen to.
