Blue Note BLP 1518, MONO Deep Groove.  Discogs lists the date of this version unknown with the key identifier being different addresses on label and jacket - label with 47 West 63rd St and jacket with 767 Lexington Ave., New York 21.  Discogs also notes a blank spine for this version.  Matrices are:
Matrix / Runout (A-side runout, etched): RVG [the Plastylite "Ear"] BN-LP-1518-A 9M Matrix / Runout (B-side runout, etched): RVG [the Plastylite "Ear"] BN-LP-1518-B 9M
Discogs also notes, for the matrices:  "Matrix / Runout (A+B side runout fragment, stamped): [the Plastylite "Ear"]"  I don't anything further on the matrix of either side on this version.
Jacket with moderate corner wear, light edge wear, split beginning top edge about 2" starting at jacket opening (although not split exactly at the jacket opening, just next to it).  Small ink marking back jacket bottom right hand corner, else no further markings, very light ring and light age-toning around edges on back jacket.  LP stored in plain white inner sleeve, LOOKS QUITE PLAYABLE with a faint scratch or scuff, I'll grade VG++.
