This is an original Scranton Mono Press with these matrices:

The jacket is the highlight of this listing. It does have an address label on front. I assume there are ways to carefully remove it including residue underneath, and if done, I would grade the jacket in EXCELLENT CONDITION. There is very light corner and edge wear, no rings or splits, a faint ink smudge near the RIAA on back jacket which is the only marking, and no age-toning of white spaces - it is a clean clear white. There is a "2" near the RIAA on back jacket. The blue original inner sleeve has a split bottom center about 3" and a second one on one side center about 2". There is another address label on Side 1 of the record, again possibly removed with little evidence it was there. This is the 3rd label variation: Side A credits the first two tracks to BMI with the remaining cuts to ASCAP. Side B credits all songs to ASCAP, except for track 5 "I Wanna Be Your Man" which is credited to BMI. The record has a clean underlay of vinyl, but also a good number of very light scratches and scuffs. I will visually grade it in the range of VG to VG+.
