Side 1/Side 2: A lot of scuffs & scratches all over the surface, a lot of feelable scratches, looks very scratchy, but still playable with some surface noise or crackle.

Matrix: "RVG" hand etched on both sides.

Front/Back: Front and back cover are completely separated, old tape on edges & spine, corner rubs.

NM NEAR MINT = Slightly less than perfect, vinyl has a few slightest of wear, nothing major.
VG+ VERY GOOD PLUS = Has a few more minor scratches and scuffs which shouldn't affect play.
VG VERY GOOD = More scratches and scuffs than VG+, but still in fairly nice condition.
G+ GOOD PLUS = Excessive wear, may have some skips or warps and usually quite noticeable surface noise.
