I always remove discs from jackets and inner sleeves for shipping to prevent seam splits, UNLESS THE LP IS SEALED OF COURSE.

MINT

Guaranteed unused, new-old-stock (N.O.S.).

"MINT" is a foolish grading term. Records are not minted like coins (neither are cars or comics). I prefer to use the term "FRESH," but I have no delusions that my objections to a decades-old system will incite a change.  

Usually only a sealed item can be guaranteed unused, and even then cannot be guaranteed to be perfect.

NM, 

NEAR MINT

A used item that looks practically new. 

An opened LP cannot be guaranteed unused. Under intense light, a few superficial sleeve scuffs might be seen deep beneath the otherwise like-new super glossy surfaces. These are okay because they will not disturb play.

In person, I actually do call this grade "NEAR FRESH."

EXCELLENT

Other dealers might use VG++ or NM-MINUS, grades which are both are misleading.

"EXCELLENT" records are highly glossy and lustrous but will have, in addition to sleeve scuffs, a few superficial hairline scratches which are noticeable only under intense light. None of these tiny defects would be felt by either a fingertip or fingernail, and they would still produce no noise during play.

Equates to "FINE" in many fields of collecting.

VERY GOOD PLUS

Personally, this grade I usually reserve for items which would be considered pretty great if they hadn't been defaced by the manufacturer and sent to the bargain bin.  You've all seen those unfortunate discs which have the extra drill hole through the label (right through the jacket, thanks a lot, Mercury!), also cut corners, saw-cuts, and punched holes. I also grade a disc VG+ if it is otherwise nice but has a small warp that does not interfere with the sound.

Equates to "ABOUT FINE" in other fields.

VERY GOOD

The first of what I refer to as a purely sugarcoated grade.  In truth, a record that is "VERY GOOD" isn't all that great to look at. However this happens to be the case with many old records, so collectors try to make themselves feel better by calling a record that isn't very good just that.  Built in overgrading.  I really like "SERVICEABLE" better.

Evident visual wear, with scuffs and scratches have left the shine in the dust. The good news is a well-made record in "VERY GOOD" condition may play much, much better than it looks especially after cleaning.

G,

GOOD

"GOOD" is the dumbest grade of all, because it is not a literal translation.

Numerous defects are allowable in this grade. Yes, the record will play through without skipping, but scuffs, scratches, edge chips, and warps, all converge into a sometimes unpleasant listening experience.

Usually also accompanied by scribbled writing, stickers, torn labels, missing labels, misapplied labels, water damage, generally any defect short of a skip or urine odor.

Doesn't sound "GOOD" to me.
