Artist: Gloria Ann Taylor  Release info: 12" 4-track single on Selector Sound - SS 3080. I purchased this from a gentleman in LA who bought this in the 80's. He said it never came with a cover. The picture shows the age as it was first purchased.  Condition:  Cover - Comes in generic sleeve a shown.
