This LP is the 1962 Walt Disney Productions LP The Story of My Teens. 
It includes the 8 page booklet, & as a bonus it comes with just the picture sleeve (no record) of a 45 release from the TV serial Annette starred in. 
The album plays with no skips & minimal scratching. 
The cover shows a bit of corner & side age wear, one partial seam split. (see pics)
