This is an original limited edition O.M.D. 10 inch single containing the songs Extended Souvenir and Motion & Heart (Amazon version), released in Canada in 1981.
The vinyl record is in mint condition. The picture cover has some wear at the extreme edges, so is graded in excellent condition.
